Yes, "Omitoufo" is just how "Amituofo" sounds to me sometimes.

I've never heard of Five refuges either - having experience with the liturgies & refuge ceremonies in Vietnamese Tiantai, Jodo Shu, and Jodo Shin Shu.

The issue of whether Samaya breakers can be reborn in the Pure Land is cut and dried from the Pure Land side. It is entirely possible. Breaking Samaya is not one of the 5 grave offenses (Ānantarika-karma) so there's not even a debate. Even if it qualified as one of the 5 grave offenses, any school ...

Sorry. Best I got is a full service. Around 12:06 there's a Namo Shakyamunaye Buddhaya chant that has a similar cadence - but not exactly the same rhythm.
