For Monastics it is one of the thirteen remainders, vow transgressions where only some of the vow remains. As opposed to something like the four defeats, where the vow would be completely broken.

Tertons can make mistakes and majorly screw up. In fact, there are many stories of Tertons making mistakes, large and small. Viz. Pema Le-dre-tsal. So recognizing someone as a valid Terton and then later also recognizing they had gone off the rails doesn't seem contradictory to me. Yes, it is true....
