In addition to those already mentioned:
TNH has to be in the current list.
Historical figures, other mentions probably include Zhiyi, Xuanzang, Milarepa, ShanTao, Saicho, Kumarajiva, Tsongkapa, Kukai, Nagarjuna, Vasubandhu, Buddhagosa, Nagasena, Asanga, etc.

Yes. Sambhogakayas are conditioned bodies, if far more pure than gross phenomenal bodies like these. This might depend on source and school.
