For something like 20+ years, my dad has been in medical limbo of some kind.
He's in the hospital now.
I have no idea how one man can spend so much time in the hospital, yet remain kind and happy.
The moment he returns home, he gets right back to hiking, beach trips, DIYing his house to make it nicer for his extended family.
He has more energy in his fingertip than I do in my entire body.
He's one of my greatest inspirations, and I wake up every day expecting him to die.
I trust in the buddhas and bodhisattvas, so I trust in the efficacy of your practice.
Please dedicate even a sliver of your Vajrasattva or Amitabha practice to him.
His name is Jan. You would like him if you met him.
May my dad be able to live in peace.
Whether he departs soon or later, may he see Amitabha and follow him to Sukhavati.
May my dad's already considerable compassion grow to include all beings in effortless bodhicitta.
May he become utterly free and save many.

Ayang Rinpoche does Liberation Prayer for the deceased every Phowa course (I think) and he did it for our Vajrasattva course. In addition, he does divinations and gives advice on how to bring deceased loved ones to Buddha Amitabha's Pure Land.

With the great compassionate lord Avalokiteśvara to his right,
And Vajrapāṇi-Mahāsthāmaprāpta on his left,
Surrounded by an assembly of countless buddhas and bodhisattvas
In the place of wonder and boundless joy and happiness
That is the heavenly realm of Sukhāvatī, the Blissful Paradise.
When the time comes for your father to leave this present life,
May he go there directly, without any other birth upon the way,
And being reborn there, may he see Amitābha face to face!
May this, my fervent prayer of aspiration,
Be blessed by all the buddhas and bodhisattvas of the ten directions
So that it is accomplished, without the slightest hindrance!
tadyathā pañcendriyāvabodhanīye svāhā

May your father receive the blessings of celestial protectors and may behold their forms with unobstructed vision.
May the rains of perfect dharma clouds ascend upon his body and may he be entirely free from any discomfort
May the sacred door of the ear be free from obstacles that he may hear the voice of manjushri
May he free living beings from imprisonment, may he offer medicines to the sick, may he provide contentment to the wretched, and wealth to the poor.
May the fruits of his good works provide him with long life, bodily ease, energetic striving, keen intelligence, and good health.
If he is not already perfectly enlightened, may he be instantly liberated with perfect enlightenment.
